Virtual Visit Platforms
Dr. Streff prefers to use the MyChart Patient Portal to perform virtual visits. In the case you're unable to sign up for a MyChart account, he will use Doxy.me platform to perform virtual visits. Both of these websites will require access to use your microphone and camera so that you and Dr. Streff can see and hear each other. Please accept requests to access your microphone and camera when prompted.
Agreement to Bill for Service
You must be physically residing within the state of Michigan during the visit in order to legally perform a virtual visit with Dr. Streff.
By accessing the link below you aknlowedge your understanding and agreement to the terms that you are within the state of Michigan and that this visit will be billed as an encounter similar to seeing Dr. Streff in office for the assessed problem(s).
